I understand and agree that, if I am employed by MVG Business LCC dba Meadow View Growers, my employment and/or compensation is entirely "at will" which means neither are guaranteed for any definite period of time, and that my employment and/or compensation can be modified or terminated, with or without cause, and regardless of the date of payment of my wages and salary, and with or without prior written notice at any time, at the option of either the Company or myself. I understand and agree that the Company reserves the right to establish and change any of the terms and conditions of my employment at its discretion at any time it deems appropriate. I understand and agree that if any previous agreements between any Company representative and myself have been made, they are superseded by the contents of this agreement. I understand and agree that no representative of the Company has any authority to enter into any agreement with me or provide me with any assurance relating to my employment with the Company, including agreements or assurance regarding the duration of my employment for any indefinite or specified period of time or to make any agreement with me or provide me with any assurances contrary to the foregoing.
I authorize the investigation of any and all of my background, qualifications and/or any other information from whomever Meadow View Growers deems appropriate or desires, as I also authorize the release of any and all information by whomever Meadow View Growers deem appropriate and desires. I also release all parties from all liability for any damage that may result from furnishing this information to Meadow View Growers. This release extends to all information deemed appropriate to be released by any requesting and/or releasing party, personal, or otherwise, as well as to the Company itself, should it find it necessary at any time to release any information regarding myself, my employment record, or my employment status to any individual or organization Meadow View Growers deems worthy of receiving such information.
I understand and agree to the fact that, if hired, I will work in a drug/alcohol free workplace which is essential to the safety of the workplace, and that I may be required to take any lawful medical exam, chemical, drug/alcohol test upon request by the Company at its sole discretion as a condition of my employment, and, if I am hired, as a condition of my continued employment at any time as deemed appropriate by the Company. I agree that my refusal to take any such tests immediately upon request may be cause for my not being hired, or, if I am hired, may be cause for immediate termination of employment. I understand that, subject to applicable law, Meadow View Growers will be the sole judge to the acceptability of any test results. I also acknowledge that Meadow View Growers is an Equal Opportunity Employer and administers its employment policies in a nondiscriminatory manner. I hereby release all persons/companies conducting such test from any and all liability.
I understand and agree that I am obligated to return in its original condition any and all Company property of any sort within my possession or control upon any demand by Meadow View Growers. If I fail to do so upon demand, the fair market value of same may be withheld, and I specially authorize Meadow View Growers to withhold said fair market value from any compensation due me, consistent with applicable laws.
I understand that Meadow View Growers implements an aggressive return to work program, in which it provides temporary, modified job assignments to accommodate workers protected by its workers compensation policy. I understand and agree to accept such assignments under this program, and direct my physician to with Meadow View Growers to provide information on any temporary physical restrictions I may have, so I may return to work safely.
I certify that the facts contained in this application and any other written statements by me are true. I agree that any misstated, incomplete, or false information is grounds for refusal to hire or immediate discharge, whenever/however discovered. I promise this because you rely on my statements in deciding to hire me. I further certify that I have read and understand all of the foregoing and hereby voluntarily agree to all provisions contained herein.
I agree that any claim or lawsuit relating to my service with Meadow View Growers must be filed no more than six (6) months after the date of the employment action that is the subject of the claim or lawsuit. I waive any statute of limitation to the contrary. If you are hired, this employment application will become part of your official employment record.
