SARA: I agree with Steven that the tree sounds like it is on the way out. If this is a sugar maple they are experiencing a serious "decline" in the state of Ohio due to environmental stresses over the past several years.
You should contact a competent arborist in your area to make an appraisal of the tree to see if there is a chance to save it and what needs to be done. MVG Plant Dr.
